INITIAL COMMENTS
 
This report is a result of a supervisory review of a complaint investigation conducted by phone on June 2, 2015, by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the supervisory review of the complaint investigation, RHD Montgomery County Methadone Center, was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.



The following deficiencies were identified during this supervisory review:
 
Plan of Correction

715.27  LICENSURE Readmissions

If a patient requests readmission to a narcotic treatment program after voluntary termination from that narcotic treatment program, that narcotic treatment program shall provide that patient with an evaluation interview and shall give that patient priority consideration for readmission.
Observations
Based on a telephone call to the Facility Director, the facility failed to provide the patient with an evaluation interview or give the patient priority consideration for readmission.



The findings include:



A call to the facility took place on August 18, 2015. The facility director informed this licensing specialist that the facility did not complete an evaluation on this patient. The facility failed to provide the patient with an evaluation interview or give the patient priority consideration for readmission.



These findings were reviewed with facility staff during the complaint investigation.
 
Plan of Correction
When a former patient who left on a voluntary basis presents to MCMC for readmission, an evaluation interview will be conducted to determine if that patient is eligible for readmission or for an appropriate referral to ensure that the patient receives treatment on the proper level of care. A BDAP form will be completed to assess for emergent care. A meeting was held on August 20,2015 by Program Director and Intake Staff to ensure that such interview is conducted. Program Director will monitor BDAP form will be reviewed for decision.
