Observations
A review of client records was completed on May 23, 2017. Based on this review, the facility failed to review and update treatment and rehabilitation plans at least every 60 days.
Client #2 was admitted on August 25, 2011. This client's most recent treatment plan update was completed April 11, 2017. The previous plan was completed January 30, 2017. In addition, a treatment plan update was completed on October 06, 2016. The previous plan was completed on July 26, 2016. Finally a treatment plan update was completed on June 10, 2016. The previous plan was completed March 08, 2016.

Plan of Correction
A clinical meeting was held by the clinical supervisor and attended by clinical staff to address treatment planning and the timeliness of due dates to be in compliance with policy and standards. Specific counselor whose plans were deficient was given a written warning. Clinical supervisor will continue to monitor each plan for compliance during supervision and prior to approval. Program Director will oversee Clinical Supervisor for compliance. Date Completed 6/2/2017 |
