This report is a result of an on-site complaint investigation conducted on March 1, 2021, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, RHD Montgomery County Methadone Center was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. |

709.93. Client records.
(a) There shall be a complete client record on an individual which includes information relative to the client's involvement with the project. This shall include, but not be limited to, the following:

Observations Based on a review of one client record, the facility failed to document a follow up within 30 days of discharge as stated in their follow up policy. Client was admitted 2/24/16 and discharged 11/23/20.

Plan of Correction As per regulation 709.93, this facility failed to document a 30 day follow up with patient. In order to correct this deficiency, MCRC Clinical Supervisor did a follow up call with the patient in question and was able to give aftercare referrals. Going forward, all counselors will conduct 30 day follow ups with their discharged patients. Clinical Supervisors will audit their charts and monitor this compliance in Individual Supervisions. Program Director will oversee this project for compliance. Time Frame: Effective Immediately |
