The Staffing Requirements Facility Summary Report for the facility was reviewed with the project/facility director on December 13, 2016. Based on this review, the facility failed to document that the project/facility director completed at least 12 clock hours of training annually.
These findings were reviewed with project/facility Director and staff during the licensing process.

Plan of Correction
The Program Director is responsible for obtaining 12 clock hours of training annually and is responsible for scheduling 12 hours of training, attending the training and obtaining a certificate of training attendance.
The Board of Directors are responsible for the overall compliance of this directive.
The Program Director is compliant with having 12 clock hours of training for the current year as of 12/21/2016. |
