INITIAL COMMENTS
 
This report is a result of an on-site licensure renewal inspection conducted on March 25-26, 2013 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, The Gate House for Men was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:
 
Plan of Correction

705.2 (4)  LICENSURE Building exterior and grounds.

705.2. Building exterior and grounds. The residential facility shall: (4) Store all trash, garbage and rubbish in noncombustible, covered containers that prevent the penetration of insects and rodents, and remove it, at least once every week.
Observations
Based on a physical plant inspection the facility failed to store all trash and garbage in noncombustible, covered containers.



The findings include:

A physical plant inspection was conducted on March 26, 2013. At approximately 2:45 PM, three bags full of trash were observed placed next to the dumpster in the rear of the facility.



The clinical supervisor was present during the inspection and witnessed the findings.
 
Plan of Correction
A larger dumpster was ordered on 4/9/13 and delivered on 4/10/13. The size was upgraded from a 3 yard dumpster to a 4 yard dumpster. To ensure all trash is placed inside the dumpster, the area will be checked daily by a staff member appointed by the Clinical Supervisor. Clinical supervisor will ensure overall compliance through observation.

705.8 (2)  LICENSURE Heating and cooling.

705.8. Heating and cooling. The residential facility: (2) May not permit in the facility heaters that are not permanently mounted or installed.
Observations
Based on a physical plant inspection, the facility failed to ensure that all heaters were permanently mounted or installed.





The findings include:

A physical plant inspection was conducted on March 26, 2013. At approximately 2:35 PM a portable heater was found atop a night stand in the "Annex Bedroom ".



The clinical supervisor was present during the inspection and witnessed the findings.
 
Plan of Correction
To ensure that portable heaters are not used by the clients, a rule was added to the client guidelines that states the use of portable heaters are not permitted in the facility. To assure that portable heaters are not brought in and used, the clinical supervisor has instructed the House Manager to perform daily house inspections.
