INITIAL COMMENTS
 
This report is a result of an on-site licensure renewal inspection conducted on March 1, 2011 through March 2, 2011 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, The Gate House for Men was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on March 29, 2011.
 
Plan of Correction

704.11(c)(2)  LICENSURE CPR CERTIFICATION

704.11. Staff development program. (c) General training requirements. (2) CPR certification and first aid training shall be provided to a sufficient number of staff persons, so that at least one person trained in these skills is onsite during the project's hours of operation.
Observations
Based on a review of personnel records, a staff schedule provided by the facility and a discussion with the clinical supervisor, the facility failed to ensure that a sufficient number of staff persons are certified in CPR and first aid training.



The findings include:



Personnel records and a staff schedule provided by the facility was reviewed on March 2, 2011. CPR and first training is provided to a sufficient number of staff so that at least one person trained in these skills is onsite during the project's hours of operation. However, the part-time driver who transports clients did not have documentation of first aid training. During a discussion with the clinical supervisor, it was noted the driver had CPR certification, but there was no documentation of first aid training.
 
Plan of Correction
The part-time driver that is cited is scheduled for first aid training March 17th. He will not be scheduled for work until his completion of First aid training is documented.



To ensure that we remain in compliance with this standard,the clinical supervisor will not schedule or permit employees to work that are involved in client coverage until CPR, First Aid, and safety courses are completed.
