This report is a result of an on-site incident investigation conducted on December 30, 2015 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site incident investigation, Malvern Institute for Psychiatric and Alcoholic Studies, Inc. was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
