INITIAL COMMENTS
 
This report is a result of an on-site complaint investigation conducted on August 4, 2022 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, St. Joseph Institute, LLC was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
 
Plan of Correction

704.12(a)(3)(i)  LICENSURE NonHosp Rehab

704.12. Full-time equivalent (FTE) maximum client/staff and client/counselor ratios. (a) General requirements. Projects shall be required to comply with the client/staff and client/counselor ratios in paragraphs (1)-(6) during primary care hours. These ratios refer to the total number of clients being treated including clients with diagnoses other than drug and alcohol addiction served in other facets of the project. Family units may be counted as one client. (3) Inpatient nonhospital treatment and rehabilitation (residential treatment and rehabilitation). (i) Projects serving adult clients shall have one FTE counselor for every eight clients.
Observations
Based on a review of the Staffing Requirements Facility Summary Report, the facility failed to keep the client/staff ratio at or below the regulation limit of 8/1. The counselor Full Time Equivalent (FTE) is determined by dividing the total number of hours devoted to active clients by the facility's work week. Then dividing the total number of active clients by the FTE. This facility has a total of 160 hours devoted to active clients with a work week of 40 hours. 160/40 = 4 (FTE). There are a total of 46 clients. 46/4 = 11.5 which equals a ratio of 11/1.
 
Plan of Correction
In July, three counselors resigned and all three put in a 2 week notice. None of the three counselors completed their 2 week notice and left the job early. By the beginning of August three new counselors were hired. One started on 8/8/2022, one on 8/15/2022 and the third counselor will start on 8/29/2022. In the future, if our client to counselor ratio is out of compliance the facility will alert the Licensure department.
