Observations
Based on a review of the unusual incident reports on February 3, 2022, the facility failed to inform the Department of an unusual incident involving an outbreak of a contagious disease requiring CDC notification within the required three days. It was discovered that the facility had an outbreak of a contagious disease, COVID-19, in January 2022 but unusual incident reports regarding staff infections were not submitted to the Department.This was reviewed with the Facility Director during the complaint investigation.

Plan of Correction
The Director of Nursing will complete an unusual incident report for every staff and client that is diagnosed with COVID-19 as well as any other outbreak of contagious disease. On 2/3/2022, the Director of Nursing completed and submitted the unusual incident reports for the seven COVID-19 cases that occurred in 2022 prior to the onsite investigation. |
