Observations Based on a review of personnel records, the facility failed to ensure that annual written staff performance evaluations, which are to be reviewed and signed by the employee, were completed in two of five applicable personnel records reviewed.
Employee # 1 was hired as the project and facility director on July 10, 2019. There was no documentation, in the record, that an annual written individual performance evaluation was completed for the reviewed year at the time of the inspection.
Employee # 2 was hired as a counselor on February 1, 2016. There was no documentation, in the record, that an annual written individual performance evaluation was completed for the reviewed year at the time of the inspection.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction Moving forward, Director of Quality Assurance and the Human Resources Administrator will work to audit HR files on a monthly basis to ensure that all employee evaluations are completed. Supervisors will be reminded of upcoming evaluations and the due date for these evaluations.
Employee #1 is scheduled to receive their evaluation on October 15, 2021.
Employee #2 is scheduled to receive their evaluation on October 7, 2021. |
