INITIAL COMMENTS
 
This report is a result of an on-site recovery house complaint investigation conducted on November 9, 2022, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Keep It Green LLC was found not to be in compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house.
 
Plan of Correction

§ 717.29(b)(3)  LICENSURE Physical plant standards - Bldg-Exterior-Grnd

§ 717.29. Physical plant standards. (b) Building interior, exterior and grounds. The licensee shall: (3) Keep the drug and alcohol recovery house free of rodent and insect infestation.
Observations
Following the physical plant inspection, it was observed that the facility failed to ensure the safety and well-being of residents as evidenced by the following:



to keep the facility free of rodent and insect infestation.
 
Plan of Correction
Akita Pest Control treated the location on 11/7/22 for roaches, and on 11/18/22 for mice. We have put the location on a quarterly treatment schedule with Akita Pest Control. The next treatment will be Monday 11/28/22 and every 3 months thereafter. House Manager will be responsible to check location once per week to be sure that problem is eradicated and no new infestations occur.

§ 717.29(g)(2)  LICENSURE Physical plant standards - Heating-Cooling

§ 717.29. Physical plant standards. (g) Heating and cooling. The licensee: (2) May not use or permit portable space heaters.
Observations
Following the physical plant inspection, it was observed that the facility failed to ensure the safety and well-being of residents as evidenced by the following:

Plan of Correction
The space heater has been removed from the location and brought to our office for storage until resident that owns it picks it up when they move. We will add "No Space Heaters" to our house rules that are gone over with and given to them at intake, and it will be noted on the upcoming house meeting notes for 11/27/2022. We will add this to the house manager checklist to be used for house checks that are done once a week at a minimum.

§ 717.30(b)(2)  LICENSURE Safety and emergency procedures - Exits

§ 717.30. Safety and emergency procedures. (b) Exits. The licensee shall: (2) Maintain a minimum of two unobstructed exits that are separated by a minimum distance of 15 feet on every floor that contains resident bedrooms. Equipment such as ladders and escapes located above the ground floor must be securely affixed to the house. Rooms, including resident bedrooms, that contain an exit must be unlocked and accessible to all residents in the event of an emergency.
Observations
Following the physical plant inspection, it was observed that the facility failed to ensure the safety and well-being of residents as evidenced by the following:



to keep the 2nd-floor emergency exit unobstructed.
 
Plan of Correction
The bed in question has been repositioned so as not to block the fire exit. We will post a "Do Not Block" sign on the fire escape ladder panel. All fire escapes will be added to the checklist that House Manager uses to do weekly house checks. This will be checked at a minimum of once a week by the House Manager.
