INITIAL COMMENTS

This report is a result of an on-site relocation inspection conducted on April 28, 2016 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Rehab After Work was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
Since this was a relocation inspection, not all regulations were reviewed. During future inspections, all regulations will be reviewed for compliance

705.23. Counseling or activity areas and office space.
The nonresidential facility shall:
(2) Maintain counseling areas with furnishings which are in good repair.

Observations
A physical plant inspection for the relocation of the facility was completed on April 28, 2016. The facility did not have any furniture in the offices.
The Facility Director confirmed the furniture was ordered, but as of the date of the inspection, had not arrived.

Plan of Correction
The Project Facility will not treat clients at the new location before an office and group room is set up on 6/1/16.
Furniture will be delivered and set up in the new location as of 6/1/16. Once the license is transferred the Project Facility will begin to treat clients. |
