Observations Based on a review of 5 personnel records, the facility failed to document an annual staff work performance evaluation, signed and dated by the employee, in 1 record reviewed.
Employee #5 was hired as a counselor on 1/16/17 and was still in the position as of the date of the inspection.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction As of September, HR will reach out to management and supervisors to begin the annual performance eval process. Clinicians will submit their self assessment and then global mastery's will be calculated to evaluated their annual performance. Clinicians will receive performance appraisals based on their ability to meet their annual goals. These will be reviewed one on one with the clinicians, signed, and date. A copy will be offered and placed with in the personnel file. This will occur mid year in May and December. |
