INITIAL COMMENTS
 
This report is a result of a renewal on-site licensure inspection conducted on April 11, 2023 by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection, 8M566701 TNH Sober Living was found to not be in compliance with the applicable chapters of 28 PA Code, Chapter 717 which pertains to Recovery House licensing.
 
Plan of Correction

§ 717.22(b)(4)  LICENSURE Beginning of Residency

§ 717.22. Beginning of residency. (b) The licensee shall complete the following documentation, which must be signed by the resident, within 24 hours of arrival: (4) Consent to residency.
Observations
Based on a review of resident records, the recovery house failed to obtain a signed Consent to Residency within 24 hours of the resident ' s arrival to the recovery house.

These findings were reviewed with the recovery house staff during the licensing process.
 
Plan of Correction
At our staff meeting on 4/17/23 the

Director (House Manager) met with

Peter Fizgerald (District Manager) and

Colin Kerrigan (Operations Manager).

During the meeting the findings were

explained. Plan of correction is as follows: Immediately following the completion of a new residents intake the House Manager (Paul Van Jura) and District Manager (Peter Fitzgerald) will review the new admits file to ensure all documentation is complete and attached to the residents profile. Additionally, the Operations Manager will review new admit files at the end of each week to ensure all files are present.
