This report is a result of an on-site complaint investigation conducted on May 12, 2021 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Progressive Medical Specialists was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. |

705.2 (2) LICENSURE Building exterior and grounds.
705.2. Building exterior and grounds.
The residential facility shall:
(2) Keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of residents, employees and visitors. The exterior of the building and the building grounds or yard shall be free of hazards.

Observations During the complaint investigation on May 12, 2021, the Program Representative was waiting in the waiting room, and observed there were several ceiling tiles that have major water stains and other tiles were missing. The exit light fixture also showed water damage.
The findings were reviewed with facility staff during the investigation.

Plan of Correction Ceiling tiles will be replaced via facility maintenance staff. Facility maintenance staff will do a walk through of the facility weekly to ensure facility structures are in tact. This will be corrected by facility maintenance staff by 6/7/21. |
705.10 (c) (3) LICENSURE Fire safety.
705.10. Fire safety.
(c) Fire extinguisher. The residential facility shall:
(3) Ensure fire extinguishers are inspected and approved annually by the local fire department or fire extinguisher company. The date of the inspection shall be indicated on the extinguisher or inspection tag. If a fire extinguisher is found to be inoperable, it shall be replaced or repaired within 48 hours of the time it was found to be inoperable.

Observations During the physical plant inspection on May 12, 2021, the facility failed to ensure the fire extinguisher had inspection tags. Three fire extinguishers are missing tags. There was one on the lower level by the confiscations room, and two fire extinguishers found on the first landing in the dining room and the end of the hallway pass the nurses ' station without tags.
The findings were reviewed with facility staff during the investigation

Plan of Correction Sticker tags have been added to the extinguishers and facility staff will create a log to sign off on to ensure signatures for fire extinguishers are up to date. Facility staff will complete weekly walk throughs of facility to ensure facility structures are in tact. |
