Observations Based on a review of personnel records, the facility failed to ensure that staff persons and volunteers received a minimum of 6 hours of HIV/AIDS and at least 4 hours of tuberculosis, sexually transmitted diseases and other health related topics training using a Department approved curriculum.
The findings include:
On November 1, 2011 six staff records were reviewed for documentation of HIV/AIDS and TB/STD training. The facility failed to document HIV/AIDS and TB/STD training in two of six records reviewed, specifically records # 3 and 4.
Employee # 3 was hired as a counselor on March 29, 2010. A Department approved curriculum in HIV/AIDS and TB/STD training was due to be completed no later than March 29, 2011. Documentation of this training was not available during the review.
Employee # 4 was hired as a counselor on May 24, 2010. A Department approved curriculum in TB/STD training was due to be completed no later than May 24, 2011. Documentation of this training was not available during the review.
Human Resources staff were interviewed on November 2, 2011 and confirmed the findings.

Plan of Correction Counselors will attend required trainings within their first year of employment. Human Resources Department has developed a training tracking grid to record trainings required and completed by employees.
All information will be communicated with Program Director on a at least quarterly basis.
BDAP approved HIV/AIDS training has been scheduled to occur at the facility December 21, 2011 for Pyramid employees.
Employee #4 has received and provided documentation of 4 hour TB/STD training completion.
Copy of certificate will be sent to licensing specialist under separate cover.
