INITIAL COMMENTS
 
This report is a result of an on-site inspection conducted for a capacity increase request. The inspection was conducted on April 5, 2011 by staff from the Division of Drug and Alcohol Program Licensure. The facility will be notified in writing of the Division's determination of the capacity increase request.
