INITIAL COMMENTS
This report is a result of an on-site initial inspection to add new activities to an existing license conducted on March 5, 2020 by staff from the Bureau of Quality Assurance for Prevention & Treatment. Based on the findings of the on-site inspection, White Deer Run York Assessment Center was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.	Plan of Correction
705.24 (3) LICENSURE Bathrooms. 705.24. Bathrooms. The nonresidential facility shall: (3) Have hot and cold water under pressure. Hot water temperature may not exceed 120F.
Observations The facility's water temperature exceeded the maximum required 120F degrees. During the onsite inspection to add activities to the existing license, the water temperature in the bathroom registered at 124F degrees.The findings was discussed with facility staff during the onsite inspection.	Plan of Correction Water heater temperature was adjusted on 3/5/2020. QI Coordinator tested water temperature on morning of 3/6/2020 and it was within required temperature, reading below 120F degrees. CEO spoke to landlord regarding requirement to maintain water temperatures below 120F degrees. Landlord agreed to keep temperature at current level of the water heater. QI Coordinator will conduct random tests on the water temperature to confirm it remains below 120F degrees.
